MIDDLE SCHOOL NURSING AND HEALTH SERVICES


  • STATE MANDATED IMMUNIZATIONS

    Beginning in 7th grade, students are required to receive the following immunizations:

    •  1 dose of Tetanus, Diphtheria, and Acellular Pertussis (TdaP)
    •  1 dose of Meningococcal Conjugate (MCV)

    The Pennsylvania Department of Health has the following immunization requirements for students Kindergarten through Grade 6.

    •  4 doses of Tetanus, Diphtheria, and Acellular Pertussis (DTaP) - First dose on or after 4th birthday
    •  4 doses of Polio (IPV) - Fourth dose on or after 4th birthday and at least six months after previous dose
    •  2 doses of Measles, Mumps, and Rubella (MMR)
    •  3 doses of Hepatitis B (Hep B)
    •  2 doses of Varicella (chickenpox) or evidence of immunity

    Please note: Compliance with all state-mandated grade level immunizations is required by the fifth day of school. If the vaccine is medically appropriate by the date of school entry and documentation of administration has not been provided, your child will be at risk for exclusion from school until vaccine requirements are met.

    You may check your child's school immunization record by logging into the parent portal, verifying with the school nurse, or by consulting with your child's primary care provider. All immunization records can be sent to the school nurse.


  • STATE MANDATED HEALTH EXAMINATIONS

    Performed by the student's private healthcare provider.

    Physical and dental examinations are mandated by the Pennsylvania Department of Education. These state required forms are due to the health office by October 15th. Examinations performed anytime after September 1st of the prior calendar year may be accepted. If you are experiencing difficulty obtaining an examination by a private health care provider, please contact your child's school nurse.

    DENTAL EXAMINATIONS: Grade 7 and all new students

    PHYSICAL EXAMINATIONS: No middle school requirement, except all new students

    Sports physicals are not part of mandated school health services but are governed by the Pennsylvania Interscholastic Athletic Association (PIAA) rules which require student athletes to receive a medical examination prior to participation in a sport, including tryouts and practice sessions. Complete information about sports physicals is supplied to students prior to participation. Any further questions should be directed to the school principal or athletic director.

  • STATE MANDATED HEALTH SCREENINGS

    Performed by the Certified School Nurse

    Height and Weight: Grades 7 through 9 (annually). You can view your child's results and body mass index (BMI) via the parent portal.

    Vision Screenings: Grades 7 through 9 (annually). If your child does not pass the vision screenings, you will be contacted and advised to follow up with your eye care professional.

    Hearing Screenings: Grade 7 (annually) and all students with an Individualized Education Plan (IEP) (annually). If your child does not pass the hearing screening, you will be contacted and advised to follow up with your physician.

    Scoliosis Screenings: Grade 7. This tests for evidence of curvature of the spine. If identified, you will be contacted and advised to follow up with your physician. If your student has a known diagnosis of scoliosis, kindly let the school nurse know.


  • MEDICATION ADMINISTRATION POLICY

    Standing Order Medication

    有料盒子 Bucks School District has a medication policy that permits our licensed nurses to administer over-the-counter medication to students through standing orders written by the district's contracted medical director. Prior to administration, annual parent/guardian permission is required by completing the Annual Student Information 有料盒子. Standing order medications include but are not limited to Acetaminophen (Tylenol), Ibuprofen (Motrin), Diphenhydramine (Benadryl), and Calcium Carbonate (Tums), cough drops, triple antibiotic cream, Caladryl, Visine, and throat spray.

    Individual Order Medication

    The 有料盒子 Bucks School District's medication policy requires authorization from both a parent/guardian and a physician for individual medication orders. In Pennsylvania, licensed nurses cannot administer medication without written instructions from a physician. Please use the form below, have it completed by your child's physician, and remember to sign and date it as the parent/guardian. Once completed, submit the form to the health office nurses. All medications must be delivered in person to the school health office and picked up at the end of the school year.
