• PROCEDURES FOR OBTAINING A WORK PERMIT AT CB WEST:
    • Work permits should be obtained at the school at which the student is attending. Permits can be obtained at all CB middle schools and high schools.

    Procedures for the School Year:
    1. Work permits are available for pick up during the school day with Mrs. Viniarski at the reception desk in the lobby. (In summer months, please go to the main office and speak with Mrs. Sidorski.  Summer office hours are M-TH 8:00am-2:30pm)
    2. Please fill out the work permit application. Include as much information as possible. You only need to fill out one half sheet. Must include a parent/guardian signature.
    3. Minor must be present to sign actual permit.
    4. Bring a form of ID/proof of age. Acceptable forms of ID include driver's license, birth certificate, passport, baptismal certificate, any other official documentary evidence indicating age/birth date of minor. (School ID does not count.)
